Registration for the IMDHA Mentoring Program can be completed both on-line and off-line. Choose your preference below.
On-Line Registration
1. Registration: Complete and submit the on-line registration form.
2. Payment: Log on to your Members Account Management and click "View Available Training" at the top menu. Click the Enroll button for IMDHA Formal Mentoring Program.
3. $500 balance is to be paid to your mentor upon assignment.
Off-Line Registration
1. Open and print the registration form.
2. Complete the form and mail, along with $75 payment, to:
